The Substance Use Disorder Counselor provides direct clinical services in General Outpatient Programs (GOP), Intensive Outpatient Programs (IOP), and High Intensive Outpatient Programs (HIOP) formally, Partial Hospitalization (PHP) programs. This role conducts assessments, develops treatment plans, facilitates groups, and maintains compliant documentation in accordance with Oregon Health Authority and Medicaid standards. The counselor collaborates with the treatment team and community partners to deliver coordinated, person-centered, trauma-informed care and supports relapse prevention and discharge planning.

The Case Manager supports individuals enrolled in substance use treatment programs by addressing barriers that impact engagement and recovery. This role conducts needs assessments related to housing, employment, medical care, insurance, and legal obligations, while coordinating with community partners and referral sources. The Case Manager assists with insurance navigation, discharge planning, and care transitions, maintains accurate documentation, and collaborates with the treatment team to promote stabilization and strengthen long-term recovery outcomes.

This role ensures accurate placement, clear clinical recommendations, and thorough documentation while collaborating with the treatment team to support each client’s path toward stabilization and long-term recovery.
